I give my permission for CrossFire Assembly volunteers to take photos of my child during VBS.
I understand that such media will only be used in promotion of the church and that no financial benefits from the use of the photos/videos will be issued. Publications may include, but are not limited to, CrossFire Assembly's website, newsletter, or brochures.
As the parent/legal guardian of the child named on the CrossFire Assembly Vacation Bible School (VBS) Registration Waiver/Release Form, permission is hereby given for my child to participate in activities sponsored by CrossFire Assembly at 3700 Coon Rapids Boulevard Coon Rapids, MN 55433.
I recognize that engaging in some activities at CrossFire may expose my child to the possibility of physical injury. I assume all responsibility for any risk.
I hereby release, and agree to hold harmless, CrossFire Assembly and any of its agents from any liability or claims arising from my chid's participation in VBS programs or related activities.
I give permission for CrossFire staff to administer basic first aid (such as a bandage) to my child, if needed.
In the event that my child should require mecical treatment, I give permission for such treatment to be administered at my cost.
At CrossFire Assembly, we seek to provide a fun and safe environment for children while we teach them God's word. In order to do this, we ask that all children participating in Vacation Bible School are at least four years old (entering Kindergarten in the fall of 2025), toilet trained, and comitted to abiding by the behavior expectations outlined below.
Please review the guidelines with your child, and encourage them to behave accordingly during their time at VBS.
*Follow directions given by Staff/Volunteers
*There will be no use of cell phones or other electronic devices during VBS, please put them away until the end of the day if you must bring these items with you
*Children are required to participate in all VBS activities (unless stipulated by their parent/guardian)
*Be considerate of others, respectful of Teachers and kind to peers
*All food items will be consumed outside during recreation time
*Candy and gum are not allowed during VBS
*Children must always be accompanied by a Teacher or another Volunteer during VBS
*Children should speak when it is their time to speak, and do so in an appropriate, respectful manner, no use of inappropriate language
*Be good Stewards, take care not to damage church property, students and/or families will be held responsible for damage incurred
Failure to abide by these expectations will result in the following:
1.) An age appropriate break from regular activities given by the class teacher
2.) A meeting with the VBS Directors
3.) A meeting with the Pastor or Deacon, VBS Directors, and parent at which point the participant will be sent home for the remainder of the week.
